Position Summary
We are seeking a dedicated and compassionate Home Health Physical Therapist Assistant (PTA) to join our growing team. The PTA will provide physical therapy services under the direction and supervision of a licensed Physical Therapist, helping patients improve mobility, strength, balance, and overall functional independence in the comfort of their homes.
The ideal candidate is patient-focused, organized, and committed to delivering high-quality care while maintaining compliance with home health regulations and documentation standards.
Key Responsibilities
- Provide physical therapy treatments as outlined in the plan of care established by the supervising Physical Therapist.
- Assist patients in improving mobility, strength, balance, endurance, and functional independence.
- Instruct and reinforce therapeutic exercises, gait training, transfer techniques, and home exercise programs.
- Observe, monitor, and document patient responses to treatment and progress toward established goals.
- Report significant changes in patient condition to the supervising Physical Therapist and interdisciplinary care team.
- Educate patients, families, and caregivers on safety techniques, exercise programs, and mobility strategies.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in case conferences and care coordination activities as required.
- Follow all agency policies, state regulations, Medicare guidelines, and professional standards of practice.
Qualifications
Required
- Current Physical Therapist Assistant (PTA) license in CA.
- Graduate of an accredited Physical Therapist Assistant program.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Business License/ EIN
Preferred
- Minimum of one year of PTA experience.
- Previous home health experience.
