Job Overview
The Clinical Pharmacist is responsible for providing safe and appropriate pharmacy services to AltaMed patients per AltaMed's policies and procedures, pharmacy practice professional standards, and applicable regulatory entities. The Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff, and other professionals regarding appropriate medication use.
Minimum Requirements
A graduate of an accredited School of Pharmacy is required.
Current, active Pharmacy license as granted by the State Board of Pharmacy
Experience working with AltaMed’s FQHC community and familiarity with issues of particular relevance to the medically underserved preferred
Bilingual in English/Spanish preferred
One of the following Minimum of 1 year of experience in sterile IV admixture procedures required, knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), Completion of an ASHP accredited Pharmacy Residency Program, Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or experience in home infusion pharmacy, or hospital setting, preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
Compensation
$53.56 - $88.63 hourly
